Donna Lucas Granger received a Bachelor of Arts degree in Psychology and Communication from Purdue University, West Lafayette, IN. Mrs. Granger has fifteen years of experience in the area of social services.
As a Case Manager at Area IV Agency on Aging, Mrs. Granger assessed, linked with services, monitored and advocated for approximately one hundred elderly and disabled clients. She determined appropriateness of nursing home placement for applicants in accordance with Indiana's Pre-Admission Screening Program.
While Social Services Representative for Visiting Nurse, Mrs. Granger assessed social, emotional, and environmental factors related to patients' health, and implemented a plan of care. She provided short-term therapy, problem solving, and long term care planning.
At Wabash Valley Hospital Community Support Program, as outpatient Case Manager, Mrs. Granger completed assessments, performed functional evaluations and developed care plans. She provided one-on-one case management to mentally ill clients. Services included teaching daily living skills, support, linkage, and symptom management. When neccessary, she arranged emergency services and crisis intervention.
